Guidance for Statin Therapy
Based on the 2018 AHA/ACC/AACVPR/AAPA/ABC/ACPM/ADA/AGS/APhA/ASPC/NLA/PCNA Guideline on the Management of Blood Cholesterol:
- ≥ 20% (High Risk): High-intensity statin to reduce LDL-C by ≥ 50%.
- 7.5% to < 20% (Intermediate Risk): Moderate-intensity statin to reduce LDL-C by 30-49%. Consider risk-enhancing factors.
- 5% to < 7.5% (Borderline Risk): Consider moderate-intensity statin if risk-enhancing factors are present.
- < 5% (Low Risk): Emphasize lifestyle to reduce risk factors.
Assumptions & Limitations
The pooled cohort equations are validated for patients aged 40-79 without clinical ASCVD. If the patient has established cardiovascular disease (prior MI, stroke, PAD), they are in a secondary prevention group and automatically require high-intensity statin therapy, rendering this score unnecessary.
