Creatinine Clearance

Estimates CrCl via the Cockcroft-Gault equation. This is the standard formula required by the FDA for adjusting drug dosages (e.g., DOACs).

Clinical Note: DOAC Dosing

While eGFR (CKD-EPI) is preferred for staging chronic kidney disease, the Cockcroft-Gault CrCl equation remains the standard for adjusting medication dosages, particularly for Direct Oral Anticoagulants (DOACs) in Atrial Fibrillation.

  • Apixaban (Eliquis): Dosage adjustment is based on a separate criteria (Age ≥ 80, Weight ≤ 60kg, Cr ≥ 1.5).
  • Rivaroxaban (Xarelto): Reduce from 20mg to 15mg daily if CrCl < 50 mL/min.
  • Dabigatran (Pradaxa): Reduce from 150mg BID to 75mg BID if CrCl 15-30 mL/min.
